Each user of this Demonstration Patient Education Web Site agrees to be bound by the terms of this Agreement.

This Patient Education Site, including the text and illustrations were developed by Health Informatics International, Inc.("H I I^TM"), its suppliers, licensors, and distributors. This site, initially established as a demonstration for meeting the health communication objectives embodied in the HealthyPeople 2010 Initiative, contains all of the available HII patient education modules. You may use the Patient Education Materials on this site only if you agree to the terms as stated below:

By selecting "I Agree" below, you represent that:

• You agree to a license limited to evaluation and review of the website materials for 30 days.
  
  
• You agree that any Patient Education Material that is reprinted will be for evaluation purposes only.
  
  
• You agree that none of the Patient Education Material will be used for any commercial purpose.
  
  
• You agree that none of the Patient Education Material will be downloaded or copied to any other computer or any digital medium.
  
  
• You agree that you will not otherwise reprint or electronically reproduce any document in part or in whole for any purpose other than than the limited number needed for evaluation purposes without prior written consent obtained from Health Informatics International, Inc.
  
  

If any provision in this Agreement is invalid or unenforceable under applicable law, the remaining provisions will continue in full force and effect. This Agreement, your rights and obligations, and all actions contemplated by this Agreement shall be governed by the laws of the United States of America and the state of California, as if the Agreement was a contract wholly entered into and wholly performed within California.
